2024 Wichita Corporate Challenge

READ ENTIRE DOCUMENT BEFORE AGREEING

LIABILITY WAIVER AND RELEASE/ASSUMPTION OF RISK. I fully comprehend the risk of personal injury and property damage that may arise from my participation and/or volunteering in any Wichita Corporate Challenge games and/or events (Corporate Challenge), and hereby agree to assume such risk including but not limited to illness, injury, or death arising from exposure to the Novel Coronavirus (COVID‐19). I further waive any and all claims of any kind that I, or my respective heirs, executors, administrators, or assigns may have or claim to have for personal injury, illness, and property damage, of any kind or character whatsoever, resulting from my participation and/or volunteering in any and all Corporate Challenge games, activities, meetings and events, against the Wichita Sports Commission, Inc. (WSC), Wichita Corporate Challenge (WCC), my employer, all sponsors, all WCC charity partners, and any event location, including but not limited to Wichita College School, City of Wichita Golf Course, City of Wichita Disc Golf course, Chicken N Pickel, Side Pockets, The Alley Bowling Center, All Star Adventures – East, and YMCA South Farha Sports Center, and all other event location owners and organizations, all participating companies and organizations, including their employees, agents and representatives, and all GWASC staff and board members, officers, games captains, other participants, and volunteers (Released Parties), and hereby release the foregoing from liability for any such claims which may arise from, or occur as a result of my participating and/or volunteering in Wichita Corporate Challenge.

INDEMNITY. In further consideration of the opportunity to participate and/or volunteer in Wichita Corporate Challenge, I hereby agree to indemnify and hold harmless the Released Parties from and against any and all claims of third parties, including but not limited to fellow employees and members of the public, for personal injury, illness, property damage, and/or claims arising from exposure to the Novel Coronavirus (COVID‐19) which may result from or be caused by my own intentional, deliberate or negligent conduct. This indemnity shall survive regardless of when such claims are asserted.

CERTIFICATION OF GOOD HEALTH. I certify that I am in good physical health, have the permission and clearance of a licensed physician to participate and/or volunteer in Wichita Corporate Challenge events, and have no physical condition, impairment, disease, infirmity, other illness, or history of chronic respiratory or circulatory ailments, including heart disease or blockage, that would prevent or jeopardize my safe participation and/or volunteering in Wichita Corporate Challenge or place me at risk of further injury, illness or death during my participation and/or volunteering.

PERMISSION TO USE NAME AND PHOTO. I hereby grant my permission to the WSC and WCC for the free use of my name and photograph in any print media, social media, broadcast, telecast, or video account of any Wichita Corporate Challenge game and/or event.

CODE OF CONDUCT. All Wichita Corporate Challenge participants and/or volunteers will:
• Conduct themselves in a sportsmanlike manner at all times, showing respect for other participants, officials, spectators, and volunteers through words and actions.
• Respect the spirit of fair‐play and non‐violence and behave accordingly in competition.
• Be responsible for their personal belongings at all times and assist in maintaining the cleanliness, safety, and security of WCC venues and equipment.
• Fully abide by the rules of WCC at any WCC‐sponsored activities. Additionally, participants and/or volunteers will conduct themselves in a manner that is consistent with the WCC mission.
• Comply with all policies and guidelines in place, including but not limited to face coverings/masks, social distancing, and host facility protocols. Participants and/or volunteers that fail to adhere to safety guidelines will be removed from an event at the discretion of the Corporate Challenge Games Director, or his or her designee

By agreeing to this form, I acknowledge that I have read the form in its entirety, fully understand and agree with its contents, and fully accept all terms,conditions, and provisions and I have not altered the form in any way. I further acknowledge by my signature that I have read the eligibility rules as stated on the WCC website, and I meet those requirements for participation and/or volunteering.
