U5 - U7s MiniRoos -Team Formation Sheet

2026 MiniRoos Team Formation sheets will only be accepted with the minimum required number of players. SUFC reserves the right to add players to teams.

Under 5, 6, and 7 teams are named as an animal. Available animal names can be viewed in the drop-down menu. Please choose your first and second preferences or select "Club Can Organise".

ALL PLAYERS must be registered.

Unregistered players are not reserved a spot in any team until registration is finalised.

Coaching and Managing appointments are subject to approval by the SUFC Committees.

The Team Representative is responsible for adding the names of Registered Players ONLY in the spaces below.

A minimum of 5 players is required to submit this form, please add SPARE in Player 6 and 7 slots, and the club will allocate additional players to fill the team up to 7 players (max).
