    Title of Textbook/Curriculum: List the titles of textbooks or resources using for instruction.

    Publisher/Website: List the publisher/website of resources using for instruction.

    Location: Indicate location of instruction (home, co-op, tutorial, on-line, college).

    Credit: Grades 9-12 indicate if class is .5 or 1.0 credit. Core courses (Math, English, Science, History, Foreign Language): yearlong/150 hours equals 1.0 credit. Non-core classes: semester/60 hours equals 0.5 credit and one-year/120 hours equals 1.0 credit.

    Course Title: Grades 9-12 indicate the course title (Algebra 1, Biology, English 9, World History...).

    Extra Curricular Activities: List extracurricular activities.

    Community Service: Grades 9-12 list community/Christian service.

      The home instruction program shall:

      (a) Provide regular, thorough instruction in the studies usually taught in the public schools to children of the same age;

      (b) Include instruction in English, mathematics, science, social studies, art, music, health, and physical education; and

      (c) Take place on a regular basis during the school year and be of sufficient duration to implement the instruction program.
