Cancellation Procedures

1. Notice Requirement:

• Month-to-Month memberships require a 30-day written notice. If cancellation notice is not received a full 30 days before the next billing date then Membership will continue for an additional 30 days before it is canceled. You will be required to pay any outstanding Fees and any Fees that will be incurred over the next 30 days at the time you provide notice of cancellation.This means that if you are billed weekly, you will make four more weekly payments after the termination form is completed. The final payment will include the use of the Studio for one week after the payment is made. If you are billed monthly, you will make one more payment after the termination form is completed. The final payment will include the use of the Studio for one month after the payment is made.

• Fixed Term memberships (3 month, 6, month, etc) will auto-renew unless canceled at least 30 days before the end of the term. If canceled with less than 30 days’ notice, billing will continue for 30 days after notice is received.

2. Fees and Penalties:

• There is no fee for cancelling a Month-to-Month membership.
• Fixed Term memberships (3 month, 6 month, etc),unless for medical reasons, military duty, or relocation, as specified in your membership agreement, you will be charged a Cancellation Fee of an amount equal to 75% of the balance of Fees due on the remaining term. The Cancelation Fee must be paid at the time of  cancellation. Documentation must be provided for these exceptions.
• Paid in Full memberships are non-refundable and non-transferable.

3. Medical or Military Cancellations:

• Supporting documentation (e.g., doctor’s note, military orders) must accompany this form. F45 reserves the right to verify the documentation provided.

4. Final Payments:

• Any outstanding fees must be paid upon submission of this form.
