RELEASE and WAIVER OF LIABILITY

I (enter name) desire to work as a volunteer for Broome County Habitat for Humanity (BCHFH) and engage in the activities related to being a volunteer. My signature below confirms that I have read this document and understand its contents.

I understand that the activities may include constructing and rehabilitating residential buildings, working in the Habitat offices and/or in the storage units/warehouses.

I understand that many of the activities performed by volunteers of BCHFH may involve a risk of injury and/or property loss or damage, including the inherent risks associated with any on-site construction activities.

I acknowledge that should I take part in construction activities, I am doing so voluntarily, and participating with knowledge of potential dangers, and I agree to assume the risks of injury or death.

I understand that BCHFH insurance policies provide that volunteers exhaust their own health and disability insurance policies prior to making a claim against BCHFH.

I grant and convey to BCHFH all right, title and interest in any and all photographic images and video or audio recordings made by Broome County Habitat for Humanity during my volunteer activities with BCHFH including, but not limited to, any royalties, proceeds or other benefits derived from such photographs or recordings.
