With applications for campers who are not students at Quest, we will need to speak to their teacher. This allows us to gain a broader perspective on your child, their behaviour and needs, and how we can best support them. 

We will be unable to accept your application without access to your child's IPP and/or psycho-educational assessment. Please accept our apologies for any inconvenience this may cause. 

Please provide a list of three additional emergency contacts who have your permission to pick up your child if we cannot contact the primary guardians. Please do not list parents/guardians from above as emergency contacts.

Please provide information about your child's mobility needs. This can be anything from dropping to the ground while walking, up to a full time wheelchair user. 

For wheelchair users, please give information on how transfers are done, and any additional posture management, such as specialized chairs, your child uses at home or at school. 

We know changes in medication and dosages may occur before the start of camp. In early June you will be sent a short questionaire to inform us of your child's current medication. After this questionaire is submitted, changes can be communicated in writing to the camp manager. 

It is best for us to have an accurate picture of your child's health and medication, particularly if we must access emergency medical care for them. We ask that you inform us of all medication your child is taking, even if it doesn't need to be administered at camp.  

Both prescription and non-prescription/over-the-counter (OTC) drugs are considered medication. This also includes vitamins, supplements, naturopathic/ homeopathic treatments, etc. 

Daily medication is taken regularly on a set schedule. This may be long-term, or short-term, ex. a course of antibiotics. 

PRN medication is taken as needed. This includes things like painkillers, allergy tablets, cough medicine, medicated creams, eye drops, etc, as well as emergency/ rescue medication such as asthma inhalers, epi-pens, behavioural aides, or epilepsy meds.

If medication needs to be given at camp it will be in adherence to our medication policy. Medication needs to be sent in its original container. Prescriptions need to be un-expired and labelled correctly. Blister packs prepared by a pharmacist for camp are strongly encouraged.

If you wish to submit any documents regarding your child's health, you may do so here. For example, doctor's letters, instructions on a particular medical condition and its management, or additional information on your child's diagnosis. 

Document uploading is optional. 

Significant: Shows considerable deficit in this area compared to expectation for his/her age. 

Moderate: Child shows some deficit in this area. 

Minor: Child has little to no difficultly in this area.
