I will receive a recurring invoice and submit payment for dues each month unless I enroll in automatic monthly payments.

Special events and performances may be offered periodically. I agree to participate in special performances and to supply my own uniforms, supplies, and transportation as needed.

I give my permission to Maui Music Mission to record images of me, and to post those images on social media and other platforms for the promotion of Maui Music Mission. I further agree to hold Maui Music Mission harmless of any and all liability or damages associated with my enrollment in the program.
