Your animal is very important to us, Two Tails Unleashed would like to assure you that every effort will be made to make your Dog's grooming experience as safe and pleasant as possible.
Liability and Safety
• Safety comes first for everyone, people as well as the dogs, during the grooming process. Accidents can occur as grooming equipment is sharp and dogs move around, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc..
• We use high quality shampoos and conditioners we have been using for years with hardly any problems but still there is always a possiblity some dogs can react badly causing skin irritation.
• Ear hair pulling is upon request only as this can be the cause of ear infections and irritation as leaving it can also cause them by holding in moisture. We do not want to be responsible for this happening but do want to make sure you are aware of this either way.
• Anal gland expression is upon request as we are not veterinarians and express them from the outside. This can cause irritation or rupturing of the gland if too full and cannot be expressed properly. We will only try lightly to express them and will not force or be responsible for problems that can occur.
• Your pet’s safety and comfort is our number one priority. In the event an accident does occur, you will be notified of the accident. If it is serious, and the owner is not on-site, we will seek immediate veterinary care for your pet. Grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the animal's owner upon signing this agreement. This release gives Two Tails Unleashed full authorization to seek medical treatment from nearest veterinarian in the case of any medical emergencies while in our care All veterinarian costs and expenses will be the responsibility of the animal's owner.
Elderly Pets
• Pets as they get in their older years can become harder to groom. With joint problems or behavioral issues, thin skin, growths and moles. We will do our best to adjust to the situation but let us know of any known medical issues, and know grooming your pet in their old age is a risk and owners are responsible for this risk.
Current Vaccinations on Pets
Pets should be up to date on Rabies and we strongly recommend they are current on distemper and Bordetella. Proof is not required but we highly recommend being up to date for your pets health safety. In knowing this we will not be responsible for sickness that could occur otherwise.
Fleas or lice
• Utah usually doesnt see many cases of fleas but has seen more lice and ticks problems. (Still rare) If your dog has traveled with you and has collected fleas, the groomer needs to be informed before appointment to ensure the safety of other pets. If lice is found during appointment. Groomer will let you know of the issue and can either turn them over to you for vet treatment or a treatment process will have to be arranged at owners expense as this can be very time consuming process with extra extreme cleaning and dog isolation to prevent spreading.
Dangerous or Aggressive Animals
• Two Tails Unleashed has the right to refuse any services at any time. In the event that your animal is too stressed or becomes dangerous to groom for either groomer or dog, we have the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and client can be charged a grooming fee (for what was done up until that point).
Use of Muzzles
• Muzzling does not harm your dogs and protects both the dog and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If an animal still acts in a way that is dangerous, we have the right to stop grooming services at any time and a service fee will be collected. We do not muzzle unless your pet gives us a reason to. Other methods are used to calm your pet, muzzling is a last resort.
Matted Coats Pets
• With severely matted coats require extra attention. Mats in a pet’s coat grow tight, and can ultimately damage and tear the dog's skin, which can provides a breeding ground for parasite infestations. We will not cause serious or undue stress to your pet by de-matting. Mats can be very difficult to remove, and may require the pet to be shaved if the groomer determines the mats are too tight to get out. Removing a heavily matted coat can cause nicks, cuts or abrasions due to skin growths trapped in the mats. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process. After- effects of mat removal procedures can include itchiness, skin redness, hematomas, self-inflicted irritations or abrasions and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments, every 4-8 weeks( most common). If your pet needs to be shaved to remove matting, by signing below, you acknowledge that you agree to this procedure, and any risk. There will be an additional charge for this process: it is time consuming, and causes extra wear and tear on grooming equipment. The additional charge can be estimated by groomer prior to appointment.
Cancellations/No call, No Show
• because we book on an hourly basis and cancellations can leave an empty block in the schedule that could have otherwise been used by another customer, we ask that any salon cancellations are made at least 24 business hours in advance. We understand that things can come up but if last minute cancelations happen more than once we have the right to refuse future service.
• Payment is due at time of pick-up. We accept venmo, cash or cards can be used thru the square app. If payment is not received. We will refuse all future services.
Your Satisfaction is important to us.
• If you are unhappy for any reason, and would like something adjusted, we will be happy to make any adjustments when you pick-up your pet from his/her appointment. However, once you take your pet home from the appointment, any return visits will be treated as a new appointment and client will be charged a grooming fee.
Photographs
• This release authorizes Two Tails Unleashed to take photos of your pet for client file and for Instagram and Facebook pages.
• I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered.
• I authorize this signed contract to be valid approval for future grooming services, permitting us to preform our services to the best of our abilities without additional signed contracts or written authorization. I understand that pricing is subject to change due to condition of dog, behavioral issues, specialty haircuts and yearly price increases.
• I have read, signed, and agreed to the above.
