Lab Coat Quote Request Form

This form is used to request an official quote for a lab coat order. When completed, we will contact you for any other details needed and send you a PDF of the quote. From there, you can send us a PO or use our credit card payment system to confirm the purchase. This form does not confirm your order. It initiates the request.
For lab coats in a pre-order phase:

Please note that they will not be delivered for at least 5 months from your order date. If a pre-order campaign is not successful, full refunds will be given. You may update or cancel your order at any time before we ship them, but please avoid this when possible. We plan manufacturing volumes around the committed orders. Large cancellations can put the entire project in jeopardy.
