  • Your FAFSA required income information that is roughly two years old. If you feel that your household income has significantly changed since then, please use this form, and supporting documentation, to present your situation. The Financial Aid Office will review each situation on an individual basis to determine if professional judgment should be utilized to recalculate financial need. We cannot guarantee an increase to the financial aid award(s), however, we do intend to be fair and timely with each case that is reviewed. The review process will generally take 2-4 weeks to complete.

  • Employment Change: Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Federal 1040 tax returns (most current, as well as 1-2 years prior)
    • W2 forms (most current, as well as 1-2 years prior)
    • Copy of last paystub from previous employer
    • Most recent paystub from new employer
    • Documentation of unemployment benefits
    • Termination letter from previous employer 
  • Death of a Parent or Spouse: Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Federal 1040 tax returns for the income year required on the FAFSA
    • W2 forms for the income year required on the FAFSA
    • Copy of Death Certificate 
  • Divorce/Separation: Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Federal 1040 tax return(s) for the income year required on the FAFSA 
    • W2 forms for both spouses for the income year required on the FAFSA 
    • Copy of Divorce decree and/or separation agreement
  • One-Time Income Distrubution: This usually results from a pay out of capital gains, IRA/pension distributions, legal settlements, etc.  Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Federal 1040 tax returns (most current, as well as 1-2 years prior)
    • Statement explaining reason for the distribution  
    • Documentation of any rollover portions
  • Excessive Medical Expenses: Only medical expenses that are in excess of 11% of your annual income and not covered by insurance can be considered. Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Federal 1040 tax return, with Schedule A included
    • Copies of applicable statements, bills and receipts
    • Statement that clearly summarizing the annual medical expenses
  • Loss of Child Support: Helpful documentation to include with this request may include the following. Please upload all that are applicable. 

    • Court documentation pertaining to the child support agreement 
    • Other relevent proof of child support loss/reduction 
  • Other: Helpful documentation to include with this request will be unique to your situation. We suggest the following:

    • Federal 1040 tax returns (most current, as well as 1-2 years prior)
    • W2 forms (most current, as well as 1-2 years prior)
