We are attorneys at the law firm of Albies, Stark & Guerriero. We are collecting information about the 2023 paycheck errors that affected State of Oregon employees.
What this form is for

Our law firm and the law firm of Bennett Hartman, LLP filed a lawsuit against the State of Oregon on behalf of a union member whose paycheck contained an error. We are collecting information about other State employees whose paychecks contained errors to find out how widespread the problem is and to prepare for a possible class action lawsuit. You do not have to fill out this form.
