K-8 ENROLLMENT APPLICATION

Thank you for applying to Presentation BVM School. We look forward to learning more about your family. In addition to completing this enrollment application, you will need to upload your child’s last two report cards, any academic evaluations/IEPs and disciplinary history (if applicable) for students entering second through eighth grades. Students entering kindergarten and first grade will need to upload prior report cards and IEPs if available. Additional information may be requested.

Before you begin, please be sure that you have PDF, JPG or PNG images of the above-noted documents available on the device that you are using to complete this application.

If there is a custody agreement in place, we will require a copy for our files.

FILE UPLOADS

For students entering second through eighth grades, upload your child(ren)'s:

• last two report cards,
• any academic evaluations/IEPs and
• disciplinary history (if applicable).

For students entering kindergarten and first grade, upload your child(ren)'s:

• prior report cards and
• IEPs if available.

Please note that if you are applying for the Parishioner Tuition Rate, you must be a registered and participating member of Presentation BVM Parish, St. Joseph Parish (Cheltenham) or St. William Parish.  A "participating member" means that you are financially contributing a minimum amount annually to one of these three parishes as determined by the pastor of the respective parish.

DELINQUENT TUITION ACCOUNTS

Student(s) may be excluded from classes for seriously delinquent accounts.  At the close of each trimester, report cards will be withheld. At the close of the school year, no academic records will be sent to other schools for any child who has outstanding tuition, CARES payments or any other financial obligations.
