Costume Order Form

I understand, I am responsible for measuring my child and selecting my child's size.  ADC will order the size I have selected herein. If the costume does not fit, I am responsible for any alterations needed and/or shipping charges to return and replace costume.  

I understand and agree to the charge on my Jackrabbit account in the amount of $85.00 (includes typical shipping charges and tax) for the purchase of this costume. Further, once the order is submitted, it is non-refundable even if my child chooses later not to participate. I further understand if I place an order after May 1, 2023 (provided this costume is still available), I will pay all applicable expedited shipping charges to ensure the costume arrives on time. 

The recomended order date is no later than March 1st.  I understand and agree it is my responsibility to place my costume order by the recomended date to ensure my child's costume arrives in time for the June 10th performance.  I further understand costume availablility and size availability is subject to change any time.  If I order late, I will accept the next size up and alter accordingly.
