Dear Parent PLUS Loan Borrower,

Complete and submit this form to continue the processing of your loan.

Due to federal regulations, all PLUS loan funds are co-payable to the College and the parent.

By completing this form and submitting it to Mid Michigan College you will be giving permission to apply the PLUS loan electronic funds transfer to your student’s account. The funds are to be applied toward all charges, such as tuition, fees, books, and supplies. Any excess PLUS loan funds will be disbursed via the method chosen on the PLUS loan application.

You have the right to rescind or modify your permission at any time. Submit any changes in writing to Mid Michigan College Financial Aid.
