I assume all risks and liability that may arise from my or my child's involvement and participation in this activity. I understand that this program carries the possibility of physical injury and may involve physical activity that may be strenuous and there are risks inherent in this recreational activity. With regard to the activity to which this form applies, nothing shall be construed to grant an expressed or implied warranty of safety. I further understand that Hidden Valley Youth Sports Association and its officers, agents, and volunteers are not liable for any injury that may result from the negligence of persons conducting this program. Hidden Valley Youth Sports Association recommends that participants secure adequate medical insurance to cover any injury that may arise from participation in Hidden Valley Youth Sports Association's recreation programs. We (Player & Parents/Guardians) further agree to abide by ALL Hidden Valley Youth Sports Association rules and to conduct ourselves in a sportsmanlike manner during all Hidden Valley Youth Sports Association related activities. We further agree to abide by all directions and decisions of the referees, coaches, and officials of the Hidden Valley Youth Sports Association Program.
I grant permission to HVYSA, N2Athletex, and Zybek to use photographs taken of my child during the event, for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
