On this form you are able to register a team (1-4 people), sign up for dinner only, sponsor a hole, and/or donate a silent auction prize! Please fill out the section(s) that are applicable to you. If you have any questions please reach out to millpondpc@gmail.com.

Please send payment via email transfer to millpondpc@gmail.com and leave a note stating your name and what your payment is for ex. "Jane Doe - Golf Team" or "Bob Mill - Hole Sponsor" or drop off cash or cheque (made out to Millpond Academy Parent Council) to the school.
