Job Description
Provides personal care and related services in the home, under the direction, instruction and
supervision of the Services Supervisor.
Tasks to be performed by a PSSW must be assigned by and performed under the supervision of a

POSITION RESPONSIBILITIES
Follows the plan of care to provide, safe, competent care/service to the service recipient.
Helps the service recipient to maintain good personal hygiene and assists in maintaining a
healthful, safe environment.
Plans and prepares nutritious meals.
Assists the service recipient with ambulation as approved.
Encourages the service recipient to become as independent as possible according to the service
plan.
Attempts to promote service recipient's mental alertness through involvement in activities of
interest.
Gives simple emotional and psychological support to the service recipient and other members of
the household and establishes a relationship with service recipient and family which transmit
trust and confidentiality.
Reports any change in the service recipient's mental or physical condition or in the home
situation to the supervisor.
Performs routine housekeeping tasks as related to a safe and comfortable environment for the
service recipient.
Prepares a visit report promptly and incorporates same in the service recipient record weekly.
Confirms on a weekly basis, the scheduling of visits so that other necessary visits by staff
members can be coordinated.
Works with personnel of other community agencies involved in the service recipient's care.
May assist with medications only if the PSSW has documented training by an RN.
Confer with the Supervisor about service recipient's status and care. Participates in the
planning of service. Attends in-service as required by agency policy.

Promptly report any change in service recipient condition to the Supervisor

JOB CONDITIONS
The ability to drive and the ability to access service recipients' homes which may not be
routinely wheelchair accessible are required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the service recipient's condition and to perform service recipient care.
On occasion, may be required to bend, stoop, reach and move service recipient weight up to 250 pounds; lift and/or carry up to 30 pounds.
Must be able to communicate clearly, both verbally and in writing.

Has access to all service recipient service records which may be discussed with the supervisor.
QUALIFICATIONS
Preferably a high school diploma or equivalent. Must be able to understand, read and write English.
Must be free from health problems that may be injurious to service recipient, self and co- workers and must present appropriate evidence to substantiate this.
Must comprehend the basics of personal care, housekeeping and meal preparation.
Must understand and respect service recipient's including ethics and confidentiality of care.
