Judicial  Bodies Expression of Interest Form

 

This Expression of Interest Form is for people who wish to be considered for appointment to the Football St George ("FSG") Judicial Bodies.

For more detail about the roles, responsibilities and functions of the Judicial Bodies, please refer to the FSG Disciplinary Regulations

Each member of a Judicial Body must have:

• The ability to exercise independent judgment; and
• The requisite knowledge and/or experience of football, FSG, Football NSW and Football Australia Rules and Regulations appropriate for membership of the relevant Body.

The Chairperson of the Appeals Committee must be either:

• a currently admitted or retired legal practitioner who has practiced continuously in New South Wales for at least five (5) years; or
• a current or retired judicial officer of a New South Wales court or tribunal; or
• suitably experienced and qualified as determined by the FSG Board.

 

In addition to setting out your experience and qualifications for the role and any other relevant information you wish to include, please provide responses to the following questions in order to assist the FSG Board to assess your nomination.

 

1. Have you been involved in activities which could, or could reasonably be perceived to, materially interfere with your ability to act in the best interests of the FSG Judicial Bodies and/or FSG?

2. The FSG Board will assess whether you are free from any interest and any relationship which could, or could reasonably be perceived to, materially interfere with your ability to act in the best interests of the FSG Judicial Bodies and/or FSG. 

3. The FSG Board may assess your commitment to and record of ethical behaviour including not having been the subject of an adverse finding or the current subject of an inquiry or investigation by any statutory, regulatory or law enforcement authority or agency including a disciplinary body of FIFA, the AFC, FA, FNSW, or FSG relating to any ethical matter.

4. Have you been involved in activities which could, or could reasonably be perceived to, materially interfere with your ability to act in the best interests of the FSG?

5. I confirm that I have no record of any ethical or disciplinary behaviour. I confirm I have not been the subject of an adverse finding and am not currently the subject of an inquiry or investigation by any statutory, regulatory or law enforcement authority or agency including a disciplinary body of FIFA, the AFC, FA, FNSW, or FSG relating to any ethical or disciplinary matter.

• The information entered above is true and accurate,
• I agree to abide by the Constitution, By-Laws, Competition Regulations, Disciplinary Regulations, other Rules and Policies of FSG, Football NSW and Football Australia,
• I agree to execute any documents necessary such as code of conduct, confidentiality, and conflict of interest.
