Thank you for applying to provide food during our event. We look forward to a good, working relationship with all the groups that wish to partner with us to put on the best events possible. In order to do that, we have a set of ground rules that we ask that you follow.

1. You are renting the cook shack, as is. You are responsible for providing your own specialty equipment to provide food and drink.  Our cook shack has a refrigerator, freezer, and stovetop range that are available to use.  Any equipment beyond that is your responsibility.
2. You must provide proof of food service permits with the State of Washington and personnel with food handler’s licenses.
3. Adults must be supervising the Cook Shack during the entire event.
4. You are responsible for the behavior and demeanor of your staff. We respect our neighbors and spectators and expect that you will also.
5. You are responsible for your own clean-up during and after your event. We will give it to you clean and we expect to get it back clean. At the end of your event, please plan a cleaning crew to police the area, empty garbage cans, bag your garbage and put it in the dumpster, or dispose of properly.
6. You will be fully responsible for any damage done to any part of the building, appliances, etc that you use during your event.

I hereby agree that I will abide by all rules of the Springdale Frontier Days Rodeo Association. Further, I hereby RELEASE HOLD HARMLESS AND INDEMNIFY the Springdale Frontier Days Rodeo Association, volunteer of the association, and any and all persons in any way connected with Springdale Frontier Days Rodeo Association from losses, damages, or injury to myself, my property and every minor person who accompanies me, resulting from participation in using this arena and grounds. I understand that any event involving horses and other animals created numerous risks of injury that are my responsibility. I understand that an animal, irrespective of its’ training and usual past behavior and characteristics, may act or react unpredictably at times based upon instinct or fright which is a risk to be assumed by me personally and I assume these risks. I, the undersigned, give the ambulance, hospital and physicians or medical staff permission to administer necessary treatment for injuries incurred while using this arena and grounds and all cost will be covered by me personally or our event insurance.

I hereby swear to the above release and unconditionally hold harmless Springdale Frontier Days Association and any/all persons anyway connected to it.
