By signing and submitting this form, I understand that group dog activities are not risk-free, including the possibility of injury and disease, and that I am responsible for all costs incurred in treating my dog, myself, and those I bring to class. I also agree attending humans and their dogs may be videotaped and/or photographed, and that Gregarious Dog Training, LLC has exclusive right of ownership and to their use without limit. I agree to keep my dog's vaccinations up-to-date while attending classes. I also agree to hold harmless Gregarious Dog Training, LLC, their owners and assigns, as well as those of the hosting facility. Course payment in required to hold my spot in class, and I also understand and agree that I forfeit all fees if I do not withdraw from class at least one week prior to its start date.
