Radiant Wellness Registered Nurses, PLLC is a PRIVATE PAY practice, so we will not submit claims on your behalf to your insurance. A non-refundable 50% deposit is due one month before services are scheduled to be provided.
We provide a minimum of 6 hours of private nursing care. All short care visits and additional services will require a travel fee and set cost. If you are unable to pay for your care prior to your procedure, Radiant Wellness Registered Nurses cannot guarantee availability for service.
The patient will receive an invoice with the breakdown of the services provided.
We may provide you with a receipt for services for you to submit to your insurance carrier for reimbursement. We cannot guarantee your carrier will reimburse you for the services provided by Radiant Wellness Registered Nurses.
Any service that is not extended care is entitled to a travel fee.
Acceptable Forms of Payment: We accept Credit Cards, Check, Apple Pay, and Zelle. As stated above, we are a private pay practice. We take payment directly from clients using our services.
Every patient is charged a $25 Supply fee, unless you have arranged for Radiant Recovery & Wellness to cover your supplies.
A $200 booking fee is charged if the service is booked less than two weeks in advance.
A $300 fee is charged for extensions of care 12 hours or more.
After 14 days of no payment of services already provided, there will be a $500 late fee charged. Every 14 days an other $500 late fee is charged.
Past Due Accounts: Patients' accounts that go unpaid for over 60 days or more may be submitted to a collections agency.
If your surgery has been postponed or rescheduled for any reason, we will accomodate this change should there be avaialbility at zero cost.
Agreement: By signing this agreement, you indicate that you have read and understand this Billing and Financial Policy. Your signature below indicates that you accept this policy and agree to abide by the terms and conditions set forth herein.
