I authorize the Academy of Canine Excellence to make decisions pertaining to my dogs immediate health and well being while in their care, including but not limited to administration of medications if deemed necessary (ie: Flea/Tick, Heartguard, CBD, allergy meds, etc). I also authorize Academy of Canine Excellence to take my dog to the vet in the case of a medical emergency or medical concern. I assume all financial liability for all fees related to my dogs health care, medications, and vet costs. *

Cancelation policy: I understand I will be charged 100% of my total stay if I cancel within 2 weeks of the start date on my booking. I understand all bookings are subject to a 50% cancelation charges (30 days prior to bookings). *

AGREEMENT TO HOLD HARMLESS, WAIVER AND ASSUMPTION OF RISK

I understand that attendance of a dog training program is not without risk to myself, members of my family or guests who may attend, and to my dog. There is an inherent risk when working with animals even when handled with the greatest amount of care, I understand that injury to my dog or other persons and/or damage to property may occur. I understand I am liable for all medical costs (both human and animal) as well as property damage caused by my dog in the course of the training, boarding, or any other services.

I am signing below to officially waive and release The Academy of Canince Excellence/Service Dogs of NM and all contract staff hereinafter referred to as the "Training Organization", its employees, officers, members and agents from any and all liability of any nature, for injury, health condition, death or damage which I or my dog may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of such damage or injury while attending any training session, boarding seervices, or any other function, of the Training Organization.
