Request To Be Included In The Munster Tennis Players Directory
This form is to be completed by a player who wishes to be included in the Munster Tennis Players Directory.

Once completed and received, and the your player grading assessement is confirmed by your coach by them completing the Grading Assessment on-line form, the relevant committee of Munster Tennis shall endeavour to update the Players Directory within 7 days.

Name of Any Secondary Club To Be Listed?
As part of your request to be listed on the Players Directory, we need to know your playing grade. This is determined by assessment by a registered coach of Tennis Coach Ireland - usually, it is a coach who is part of your club.

After your assessment, the coach will complete a form on your behalf (which you can view at this link) and they will confirm your ITN and Tennis Ireland grade. Once we receive the assessment, we shall endeavour to update the Players Directory within 7 days.

Should we need to follow up your assessment, we ask you for the name of your coach.

It is your responsibility to arrange the time and venue of your assessment and any costs that may be requested and ensure they complete the assessment form ahead of you being added to the Players Directory.
