Take into consideration that this registration contains sections that will require electronic signatures (esignatures) that can only be placed by the participant of the REC. If you are filling out this form on behalf of someone else who is older than 18 years of age (adult), we recommend that the participant is with you while filling out the form so he is able to place the esignatures. If you are filling out this form on behalf of a minor, you must be the parent or the legal guardian of the minor for whom this form is being filled out

A traves de este enunciado certifico lo siquiente:

1- Que soy el participante del REC por el cual se esta llenando este formulario O QUE soy el padre o representante o tengo la custodia legal del participante si es que este es un menor.

2- No estoy firmando los siguientes formularios en nombre de alguien mas, a menos que este llenando dichos formularios en nombre de un menor del cual soy padre o sobre el cual tengo la custodia legal.

3- Tengo la autoridad legal de firmar dichos formularios.

 

Estos formularios y todas sus secciones cumplen con los Procedimientos de Seguridad y estatutos especificados en Los Estatus del Estado de New Jersey de acuerdo al articulo 40 de Uniformidad de Transacciones Electronica Secion 12A:3-402 y cualquier otro Estado o Ley Federal referentes a firmas digitales (esignatures)

2- I am not signing the following forms on behalf of anybody else unless I am signing the minor participant, for which I am Parent or legal guardian.

3- I am legally authorized to sign this form.

 

It is intended that this form complies with the Security Procedures and the statutes under New Jersey Act 40 Uniform Electronic Transaction Act (2000 - 152, s.1) and any other State or Federal Law with regards to signatures.
