• 2023 #AZsportsHOF Ballot Nomination Form

    This is the official nomination form for the Arizona Sports Hall of Fame. To nominate someone for the Arizona Sports Hall of Fame you must submit their name during the nomination period. All nominations are verified to ensure that the nominee fits the Eligibility Criteria required by the Hall.  Once verified, the nominee is presented to the Arizona Sports Hall of Fame Trustee Board Members. The members will vote to determine which nominees are placed onto the ballot.  The 2022 ballot will open for voting on April 1, 2022 and run through April 15, 2022.  All nominations are collected electronically through the www.azsportshof.com website.

  • AZHOF ELIGIBILITY CRITERIA

    The candidate shall be an individual who has merited recognition and distinction in sports and who has brought fame and honor to the State of Arizona through outstanding sporting accomplishments or contributions. Candidates are eligible through any one (1) of three (3) criteria:

    1. A native of Arizona
    2. Immediately recognized as an Arizonian
    3. Have made at least two significant contributions in    the athletic community in the State of Arizona.

    The Arizona Sports Hall of Fame Trustee Board Members have the final say and responsibility on who is inducted into the AZHOF.
