Daycare Permission Slip

While your pet(s) are participating in group daycare, there is a chance that your pet could get injured (bit, scratched, bruised, etc...) Four Paws Inn will not be responsible for any injuries or vet bills, nor will any other daycare pet owners be responsible. Dogs will be dogs and sometimes they play rough.

Four Paws Inn will only allow pets that get along with each other to play together. Any pet that does not get along with others or starts fights will be removed from the group. If a timeout from the group does not work, the pet will have individual time and will not be allowed in group play. Only pets that are spayed or neutered are allowed to participate in group play.

I understand that Four Paws Inn will not be responsible for any injury, illness, or escape. I hereby grant permission to Four Paws Inn to act on my behalf in case medical assitance is needed. In such case, the pet will be taken to Missouri Valley Vet Clinic and I will be responsible for the cost of the bill. I understand that Four Paws Inn will do all that they can to prevent such injury, illness, or escape.
