Pre-K Intake Form: Fall/Spring 2025-2026
This intake form is for students who do not need wrap-around care and
will only attend during Pre-K Hours: Tuesday-Friday from 7:50-11am
1. Before completing this intake form please note that:
  - ALL Pre-K eligible students in the VASD MUST register with the VASD first.
  - Pre-K placement is assigned by the VASD.
  - Completing this intake form does not guarantee placement.

2. Legacy Families of The Caring Center/VMH:
  - You must register with the VASD first.
  - It is your responsibility to confirm/report your Legacy Status with/to us.
  - Please contact us if you have not already done so.

  1. Register with the VASD first.
  2. Complete The Caring Center's registration form.
Some helful info:
- Your child is eligible to attend Pre-K if they are 4 years old by/before Sept 1, 2025
- VASD = Verona Area School District
- VMH = Verona Montessori House
- Wrap-Around Care = Care before and/or after 7:50am - 11am
- Legacy family = You have a child that currently or previously attended.
