ONE LODGING UNIT PER LODGING REQUEST FORM:

Tulsa Artist Fellowship offers complimentary lodging, when available, in one of our two apartment units located in the Archer Building for program-related events. Lodging is provided on a first-come, first-served basis. In some cases, hotel rooms may be provided depending on the program and availability.

It is important that the lodging guest complete this form themselves whenever possible. If a staff member or third party submits on their behalf, all questions in the form must be reviewed with and relayed to the guest in advance. This ensures the guest knows what the apartments offer and that their needs are met before arrival.

This form is intended to request one lodging unit. Please complete a form for each separate lodging request. If more than one (1) guest is to stay in the same unit, the same form can be used for that lodging request. However, in the "Additional Information" Section at the bottom of the form, please specify if those guests need:

a) separate beds

b) separate bedrooms
