1) Attending sessions and agreeing to inform my practitioner promptly if cannot attend a session, in accordance with the cancellation policy.
2) Setting my own personal wellness objectives and goals.
3) Voicing my opinions, thoughts, and feelings honestly and openly with my practitioner, both positive AND negative feelings. ESPECIALLY the negative ones!
4) Being actively involved and engaged during sessions.
5) Following through with assigned or suggested practices, homework and/ or exercises/ activities in- between sessions. AND, If unable to complete these activities being open and ready to discuss barriers to completion, and being willing to work through these barriers, to complete the assigned or suggested practices, homework and/ or exercises/ activities
6) Taking my medication as prescribed by my physician. Or, if want a medication change, dosage change, or want to discontinue any of my medications I will do this under the advisement and treatment of my physician.
7) Being willing to branch out and attempt new behaviors, new ways of being with myself, others, and the world at large.
8) Implementing a crisis response plan when needed.
9) Provide information about other treatments and treatment providers that may impact my treatment here. This may include medication records, other diagnoses, and other counseling or case management services.
10) I realize that no matter what my current circumstances, past experiences, and/ or triggers, I am ultimately responsible for my own behavior and agree to take accountability for such.
I also understand and acknowledge that, to a large degree, a successful treatment outcome depends on the amount of energy and effort I put into the process. I understand that my practitioner cannot do the work for me. If feel that the treatment plan is not working, I agree to discuss this with my practitioner, and attempt to come to a mutual understanding regarding what is not working, and to identify potential solutions, to determine if a mutually agreeable resolution is an option. I understand that my practitioner's primary motivation is to help me achieve all my mind, body, and spirit wellness goals, and it will not upset them, or hurt their feelings, if ask them to help me find an alternative treatment provider, if doing so is what I desire and/ or is in my best interest. In short, I agree to make a commitment to living.
This agreement will apply for the duration of treatment and our work together. The treatment plan can also be reviewed and modified at any other point, at my request or at my practitioner's discretion and prompting.
