1. Are you at lease 18 years old? Yes / No Important: the minimum age to volunteer is 12. Volunteers between the age or 12 and 16 must be accompanied by a parent Volunteers 17-18 must have a parent or guardian waiver on file NO EXCEPTIONS

 

7. Because the Hutchinson Animal Shelter is a subsidiary of the Hutchinson Police Department. Some volunteers may require a Background Check. To do that please Provide the Following:

We appreciate your interest in the City of Hutchinson's Animal Shelter, and in the animals we serve. HAS strives to ensure the safety of all volunteers. However our work may occasionally involve potentially hazardous situa- tions. We handle animals whose behavior and health backgrounds are unknown. The care and maintenance of the animals and our facility often involves procedures and equipment that could be hazardous. HAS urges all volunteers to take precautionary measures to avoid personal injury or property damage be careful and to obey all reasonable orders of supervisory staff concerning your work.

Iam eighteen (18) year of age or older and in consideration of being permitted to volunteer with HAS, for myself, my spouse, my heirs, executors, administrators and assigns, hereby release and forever discharge the Hutchinson Animal Shelter, its heirs, administrators, and executors, successors, and assigns, of and from any and every claim, demand, action,or right of action, of whatever kind of nature, either in law or in equity, arising from or by reason of accident that may occur as a result of my participation in the activities of Release or any activities in connection with release, whether by negligence or not.

Ifurther release all officials and professional personnel of HAS from any claim whatsoever on account of first aid, treatment or service rendered to me during my participation in the activities of HAS

9. Minor Consent: Ihereby consent that my minor child may act as a Hutchinson Animal Shelter (HAS) Volunteer. I understand that injuries may occur and that HAS is not responsible for any damages which my result from my child's work for HAS and 1 assume all risk of damage for my child. In giving such parental consent, I agree that this consent will operate as a bar to recover all suits, demands, claims, or other actions which may be brought by me or any person on behalf of my minor child, for damages resulting from injuries suffered by my minor child while acting as a volunteer for HAS. Furthermore, I understand that this agreement is a bar to recovery by me for damages claimed in the event damages are sustained by me as a result of my child's injury. Finally, I shall indemnify HAS and its successors, assigns, and insurers against loss or expense with the respect to any claim, action or demand by any person on behalf of my minor child based on any injuries which may be sustained by my minor child while acting as a volunteer for HAS.

10. Volunteer Terms and Conditions

By Signing Below, I hereby accept a position as a volunteer for the Hutchinson Animal Shelter, (HAS) upon the following terms, conditions and understandings: My services to HAS are provided strictly in a voluntary capacity, as a Volunteer and without any expressed or implied promise of a salary, or compensation of any kind whatsoever.

I agree to hold absolutely confidential all information that I may obtain, directly or indirectly, concerning clients and staff. I agree not to seek to obtain confidential information from a client. I understand that an intentional or unintentional violation of confidentiality may result in disciplinary actions, including termination by the shelter and or possible legal action by clients/customers.

My services are furnished without any employment-type benefits, including, employment insurance programs, workers compensations accrual, vacations or sick time.

I will familiarize myself and comply with the HAS policies and procedures applicable to Volunteers. I understand that HAS expects high standards of moral and ethical treatment of the animals under its care.

I understand that HAS can and with our notice or hearing terminate my services as a volunteer at any time, with or without reason.

I will be punctual and conscientious, conduct myself with dignity, courtesy, and consideration for others, and endeavor to make my work professional in quality.

I will furnish and maintain appropriate attire and maintain a well groomed appearance for my volunteer assign- ments.

I will carry out my assignments and seek assistance from the job supervisor when necessary.

Iwill adhere to the department's sign in procedure and notify the volunteer coordinator or job supervisor if am unable to work as scheduled or want to discontinue my volunteer service at the shelter.
