Instructions: Rate the employee’s performance during the review period by checking the most appropriate numerical value in each section. Be sure to add comments below to support the numerical value.
Section I-Key Evaluation Criteria: Applies to all staff level employees and should be completed for all staff positions.
Section II- Goals: Applies to all staff level employees including those who manage a team.
Rating and Scoring: To determine the overall performance score, the numerical values of Section I will be automatically calculated for a final score.
Section II is for current and future goal setting only and no performance rating.
Prior to the performance discussion with the employee, a detailed plan to address areas rated “below standards” or “unsatisfactory” must be submitted to human resources for review. A plan could include a formal written Performance Opportunity Plan or a professional, departmental or developmental goal included in the Performance Appraisal.
Ratings are based on the following scale:
5 Outstanding- Performance and results achieved always exceed the standards and expectations of the position requirements, performance standards and objectives.
4 Exceeds Standards- Performance and results achieved consistently exceeds the standards and expectations for the position requirements, performance standards, and objectives
3 Meets Standards- Performance and results achieved consistently meets the standards and expectations for the position requirements, performance standards, and objectives
2 Below Standards- Performance and results achieved generally do not meet the standards and expectations for the position requirements, performance standards, and objectives
1 Unsatisfactory- Performance and results achieved consistently do not meet the standards and expectations for the position requirements, performance standards, and objectives
