《收集個人資料聲明》
參加者在報名表格上提供的個人資料，會由消費者委員會（消委會）收集及處理，並只用作安排及處理消費權益講座及消費者教育的相關活動。消委會承諾會全力遵守《個人資料（私隱）條例》的規定，亦會確保屬下職員及第三方服務供應商遵循有關規定。消委會採取適當步驟以保障所持的個人資料免受喪失、未經准許的查閱、使用、修改或披露。參加者有權查閱及更正所提供的個人資料。更正資料要求應以書面向消委會教育部計劃支援主任提出，查詢請致電2856 8573。消委會可能酌情就複印資料收取行政費用。

Personal data/information collected in this form will be used only for the purpose of processing and making arrangements for the talks and relevant consumer educational activities. The Council is committed to protecting the privacy, confidentiality and security of the personal data we hold by complying with the requirements of Personal Data (Privacy) Ordinance with respect to the management of personal data/information. The Council is equally committed to ensuring that all our employees and agents uphold these obligations. You have the right to request access to and correction of personal data/information held by us about you. Request for access to or correction of such information should be made to the Project Support Officer, Consumer Education Division in writing. For enquiry, please call 2856 8573. A fee may be charged for supplying copies of related information.
