Personal Training Agreement and Terms of Service
Congratulations on your decision to participate in a personal training program! With the help of your trainer, you are investing in your health, strength, and overall wellness. Your consistent participation will greatly improve your ability to accomplish your fitness goals faster, safer, and with maximum benefits. The habits and techniques you learn will stay with you for life.
In order to maximize progress, it is important that you follow the program guidelines during both supervised and (if applicable) unsupervised training days. Remember: consistent exercise and healthy nutrition are essential for the best results. While every effort will be made to ensure your safety during training, you understand that there are inherent risks involved with any physical activity, including increased heart stress and the possibility of musculoskeletal injuries.
By voluntarily participating in this program, you agree to assume all risks and waive any claim for personal injury or damages against myElevation Wellness, its trainers, and affiliates.
You further affirm that, to your knowledge, you have no limiting physical conditions or disabilities that would preclude participation in a fitness program.
By signing below, you acknowledge and accept full responsibility for your health and well-being during your participation, and agree that no responsibility is assumed by the leaders or trainers of this program.
Personal Training Terms and Conditions
1. Session Scheduling and Cancellation Policy
All sessions must be scheduled in advance. Sessions canceled or rescheduled with less than 3 hours’ notice will result in forfeiture of that session and will incur a $10 late cancellation fee. No-call, no-show participants will also be subject to the same policy.
Outstanding invoices must be resolved before the next session or subscription billing cycle. Failure to do so may result in account suspension or additional fees.
2. Late Arrival Policy
Clients arriving late will receive only the remaining scheduled session time. Clients arriving more than 10 minutes late will not be permitted to participate in that session.
3. Session Expiration Policy
All purchased personal training sessions must be used within 90 days from the purchase date. Sessions not used within this period will be considered expired and void.
4. Refund Policy
No refunds will be issued for any reason, including but not limited to relocation, illness, scheduling conflicts, or unused sessions.
5. Subscription and Payment Terms
Clients enrolled in monthly subscription plans have five (5) business days from the payment due date to cancel or modify their subscription. Missed payments will result in suspension of training services until the account is brought current. Trainers reserve the right to terminate services for non-payment or repeated payment issues.
6. Notice of Termination
If a client chooses to discontinue training, they must notify their trainer immediately. Failure to formally cancel may result in additional fees or ineligibility to continue a training relationship.
Program Description
The personal training program options are as follows:
• Bootcamp Sessions:
Group-based sessions designed for conditioning, strength, and endurance. Each bootcamp session is approximately 45 minutes.
• Semi-Private Personal Training Sessions:
Small group personal training focused on individualized attention within a shared environment. Each semi-private session is approximately 60 minutes.
• One-to-One Personal Training Sessions:
Private training sessions with a fully customized approach. Sessions range from 30, 45, or 60 minutes, depending on the package purchased.
Participants are encouraged to commit to consistent attendance and effort to maximize results and achieve their personal fitness goals.
Please Note:
• Bootcamp sessions are approximately 45 minutes in length.
• Semi-private personal training sessions are approximately 60 minutes in length.
• All credit and debit cards are securely stored through an encrypted payment system via Square.
• Regular communication and adherence to scheduling and payment policies ensure the best possible training experience.

By signing below, I acknowledge that I have read, understood, and agreed to the terms outlined in this Personal Training Agreement. I understand the expectations and my responsibilities as a participant in the myElevation Wellness training program.
