This checklist addresses responsibilities and possible safety hazards which may be present at a location. This self-survey is to be completed by anyone who has access to the location. Upon completion, the form should be provided to all managers and supervisors at the location and it is kept on file. Once completed, this will go into our database too and can be provided anytime.  You will also get a email copy of your survey.
