  • Please complete the following form if you intend to adopt a cat from Community Cat Angels

  • I understand and agree that the animal I am adopting is a rescue animal and that the animal may respond to situations unpredictably. I understand and agree that Community Cat Angels makes no representations or warranties, express or implied, about the animal's health, behavior or temperament. *

    If at a later date I am unable or unwilling to keep the animal, I agree to contact Community Cat Angels and return the animal at no charge to Community Cat Angels. I agree that I will not relinquish the animal to another person or group without first letting Community Cat Angels know that the animal will be rehomed. This enables Community Cat Angels to keep appropriate records of their adopted cats. Community Cat Angels expects a reasonable amount of time; seven (7) calendar days from date of written notice to respond. This gives Community Cat Angels adequate time to notify their internal members and give them a chance to act.

  • I agree to fully and completely release Community Cat Angels, its officers, directors, volunteers, agents, and representatives, from any liability of any kind whatsoever arising from any health or temperament issues the animal may have or may develop, and/or from any and all claims relating to my care, ownership or possession of the animal    *   

  • I agree that I will not have the cat declawed.      *    CCA does not allow an adopted cat to be declawed! A violation of this rule will disqualify you from adopting!


    I agree to indemnify and hold harmless Community Cat Angels, its officers, directors, volunteers, agents and representatives from any and all losses, fines, claims or damages, including attorneys' fees, arising out of or relating to my care, ownership or possession of the animal, including any claims for injury or damage to persons or property caused by the animal.   *   

  • I also understand that I am adopting a rescue animal and that this animal may not have been examined by a veterinarian. I am accepting the animal as-is without knowing any previous medical history. I understand that Community Cat Angels is not a medical/veterinarian organization and that they are Good Samaritans who volunteer their time trying to find abandoned street cats a safe and loving home. I am also taking full responsibility to have the animal examined with the recommended vaccinations including Rabies, spay/neuter and micro-chip performed.   *          

  •  I certify that I am at least 18 years of age and fully understand and agree to the above statements that I have initialed. I am taking full responsibility for the animal upon initialing and signing this document.

  • Thank you for choosing to adopt a rescue animal.  An animal is a friend for life and we want to assist the potential adopter in finding the family member that is most compatible with his/her lifestyle. An unwise adoption can result in an unpleasant experience for not only the adoptive families but for the animal as well. We hope you will agree that the animal's welfare must be our main consideration. If an adoption does not work out, we will expect to be notified immediately and will gladly take our animal.

    Please complete the following form if you would like to adopt a cat from Community Cat Angels.  In order to be considered for adoption, the following is required:

    • Proof that you are at least 18 years of age
    • A copy of your state issued driver's license. If you do not have a driver's license, then a copy of your state issued photo ID with proof of current address (e.g. utility bill)
    • Proof of home ownership or approval to own a pet from your landlord/property management

    MEDIA RELEASE FORM

  • I,   *  , grant permission to Community Cat Angels, a 501 (c)3 Organization, hereinafter known as “CCA”, to use my image (photographs, videos and/or statements) for use in CCA publications including:  

  • I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and that I waive any right to royalties or other compensation arising from or related to the use of the image.
