Pet Care Agreement
This agreement includes a release of liability and waiver of legal rights and deprives you of the right to sue the owner of Fido’s Bed n Biscuit LLC. Do not sign this agreement unless you have read and understand it in its entirety. Seek the advice of legal counsel if you are unsure of its effect.

1. All dogs must be current on Rabies, Distemper, Parvo, Bordetella, and any other required vaccinations. Records must be provided to Fido’s Bed n Biscuit and updated as needed.

2. All dogs must be on flea, tick, and heart worm medications 12 months a year. They must also be free of ticks and fleas and in good general health.

3. Be non-aggressive towards dogs and people. All dogs will be kept together of all sizes and cannot be separated. If your dog becomes aggressive you will be asked to come get your dog.

4. Dogs must be able to be left alone. Wren (Fido’s Bed n Biscuit LLC owner) will never leave dogs alone more than 4 hours at a time. Wren will make best efforts to have a contractor fill in when possible.

5. All dogs must be wearing a collar with their owner’s information on the collar when being boarded or at daycare.

6. Any damage to property of Fido’s Bed n Biscuit LLC estimated by Wren to be over $50 damaged by your dog will be replaced or paid for by the owner of said dog. Wren will determine which animal did such damage and the owner will reimburse for said amount.

7. Dogs will be fed twice a day (breakfast and dinner). If your dog needs an extra feeding this may have to be split between the two meals. All dogs are fed together unless there is food aggression at which time you must let Wren know ahead of time so she can make appropriate accommodations.

8. You are welcome to bring toys, beds, and other personal items but they may not be returned to you, or they may be damaged. Other dogs may destroy them.

9. Duty to disclose: You, as the owner, must disclose on a continuing basis any and all medical or other conditions, including but not limited to personality concerns or behavioral incidents that may affect your animal’s ability to be around others.

10. Escape behaviors must be disclosed. This includes fence jumping, digging, darting out doors, etc. Fido’s Bed n Biscuit LLC takes precautions for these behaviors but cannot be liable if your dog does escape or get injured.

11. Dogs may be given CBD treats if they are anxious. This is at the discretion of Wren. If your dog is anxious about storms or loud noises, it may be beneficial to talk to your vet about medication for when they are boarding.

12. I authorize Wren Armstrong and Fido’s Bed n Biscuit LLC to act as my agent in the event of my animal(s) needing medical attention. I further agree that I will be financially responsible for all veterinary care deemed necessary by the licensed Veterinarian.

13. Hours are 7am to 9 pm daily (except for major holidays) BY APPOINTMENT. Owner must text Wren at 417-459-1765 PRIOR to coming to Fido’s Bed n Biscuit LLC.

Temporary Pet Guardianship Form

Accidents happen and this form provides Fido’s Bed n Biscuit LLC someone to contact in the unlikely event you don’t come home. This does not take the place of anything written in a will but allows someone local to take guardianship of the animal(s) until legal guardianship is granted by the courts.
