CGSUSA Submit Photos to Annual Journal and Sign Release

I hereby certify that I am the parent or guardian of the above-­­­named minor child, and warrant that I have the legal authority to execute this consent form on behalf of this child. I hereby grant the United States Association of The Catechesis of the Good Shepherd (CGSUSA) the right to use my child’s writing and/or artwork, or my child’s photographic image, on its website or in CGSUSA publications. I understand that while any children appearing in photos must be fully identified on this form, any captions that appear on the website will not identify children by name. I understand that my child’s first name, age, and the location of his or her atrium will be used to identify my child’s writing and/or artwork. I also understand that a limited amount of background information provided by the catechist—e.g., presentations recently seen in the atrium or my child’s comments about the work—may be printed in connection with the publication of my child’s writing and/or artwork. I agree that if my child’s work or photographic image is not used on the website or in a publication, it may remain on file with CGSUSA for possible use at a future time.

I represent and warrant that I am the sole owner of all rights in and to the photo(s) submitted, that the photo(s) is/are original, that the photo(s) does/do not infringe upon any copyright, proprietary right or privacy right of any individual or organization, and that I have the right to enter into this agreement. I shall indemnify CGSUSA and hold CGSUSA harmless against any and all loss, damages and expenses that CGSUSA may suffer by reason of any claim arising from the breach of any of these representations or warranties made by me. I understand that CGSUSA shall be the owner of copyright to its website and any and all publications. I agree that CGSUSA shall have final editorial control over any and all material used in its website or publications, and that all proceeds derived from the website, the sale or distribution of the publications shall be the sole property of CGSUSA. I agree that if my photo is not used on the website or in a publication, it may remain on file with CGSUSA for possible use at a future time.
