Time to Create Your Custom Maternity Photography Studio Session

Maternity Session:

We have a wide assortment of dresses available at each studio, ranging from size S to XXL. You are also welcome to bring any dresses or clothing that you would like to use with you. For husband/family members we recommend wearing neutral or white colored shirts or T-shirt with pants or jeans. We also have a selection of 2T, 3T and 4T dresses, however it is not as wide at maternity gowns assortment and we recommend to bring your outfits for kids. Hair & makeup is not included in package, however we do recommend having them professionally done for your session, as it adds to the beauty of your images.  The session duration may vary depending on your package, and as sometimes we have several session scheduled, please plan to arrive to the studio at least 10 minutes in advance.

*If you prefer to do an intimate style photo set, please bring several sets of undergarments with you (neutral tones, white, black, etc). If you would like to do artistic nude images, please let photographer know and we will ensure your privacy.

Your Maternity Session Special includes: 35 minute studio session with 2 sets. Viewing gallery with up to 25 edited gallery images for purchasing digital and printed art.  (This fee is included in the promotional special you ordered.. If you have not made full payment for your special, you will be required to pay separately from this form prior to your shoot)

It is understood that 4Baby Photography (Photographer) is the exclusive, official photographer retained to perform photographic services requested on this contract.
Photographer reserves the right to use negatives, RAW images, edited images and/or reproductions for advertising, display, publication or other purposes, not restricted to use on the web, Facebook, business cards, or posters.
No part of any order, including previews, will be delivered until the balance is paid in full. Payment is required in full at the time of placing an order. Prints will not be ordered until payment is made.
Photographer retains the right to edit the photographs and omit any image. It is understood that Photographer will not deliver every exposure taken. Client agrees to abide by Photographer’s editing decisions.

Client shall assist and cooperate with Photographer in obtaining desired photographs. Photographer shall not be responsible for photographs not taken as a result of Client’s failure to provide reasonable assistance or cooperation. Client will be respectful to Photographer and all parties being photographed.
Client will not hold Photographer or the owner of the property or any other agent or representative of the company liable for any injury that may occur during the session.
The charges in this Contract are based on Photographer’s Standard Price List. This price list is adjusted periodically and future orders shall be charged at the prices in effect at the time the order is placed.
In the event Photographer files suit to enforce any term or condition herein, Photographer is entitled to expenses of litigation, including reasonable attorney fees.
All retainers and payments are non-refundable.

No Shows/Tardiness

• Photographer understands weather, illness, or other reasons may cause Client to reschedule a session. Client will not be charged a penalty rescheduling a session provided at least a 24 HOURS notice is given. Client may reschedule the session, at the convenience of Photographer, without being charged a fee.
   
• No show or rescheduling within 24 hours will incur a $75 reschedule fee at the discretion of Photographer. Photographer will make every effort to spend the necessary time, including on multiple days, at the convenience of Photographer. Photographer may reschedule without penalty.
   
• If the client is running late to session, after 30 minutes, session will be canceled and/or rescheduled to a different date and time at the photographers discretion and a fee of $75 will be charged to client. Any other occasions of tardiness, session times will not be extended.
   

Session Protocols

• To make for a better sesion, no adult or child besides the parents of the baby are allowed in the studio at any time until their time is requested by the photographer to take part in the studio session.
• Any additional session fees for additional family members to take part in the shoot will be paid prior to the photo session.

Phones, No Cameras and Video Camera Rules

We work hard to create session sets and edit our images, so we do not allow any pictures or video of any kind  taken during the session. 

• Phones are allowed for text and communication purposes.
• NO Cameras or Video Cameras allowed in the studio.
• If any image or video is taken during the session, the session will end without any refund, and there will be an additional fee of $600. 

RELEASE AND WAIVER OF LIABILITY I, HAVING AGREED TO ACT AS A MODEL, HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.  I certify that there are no health-related reasons or problems, which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:                                                                    
1)I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the ordinary negligence of the provider or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: 4Baby Photography and/ or their owners, directors, officers, employees, representatives, insurance carrier, and agents;

2)I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in the above paragraph from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of such entities or persons or otherwise.

I acknowledge that 4Baby Photography and their owners, directors, officers, employees, representatives, insurance carrier, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of 4Baby Photography.

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
