Golf Tournament Information:

DATE: Monday July 24, 2023. 9:00am Shotgun start. Come at 8:00am for check-in. Includes Breakfast Burrito & Smokehouse Buffet Lunch.

PLACE: Perry Park Country Club, Private Golf Course, 7047 Perry Park Blvd, Larkspur, CO 80118

FORMAT: Four-player scramble, mulligans for sale. Standard scoring, no handicap.

ENTRY FEES: $185 per player (advance payment guarantees a spot)

HOW TO ENTER: Complete the form and return it with $185/player or $740 per four-some.

*Fewer than 4 players are welcome and will be grouped.
