Service Agreement

My Midwives provides an antenatal course for pregnant women and her support person.  The course is designed to provide information and education pertaining to pregnancy, labour, birth and the postnatal period.  We understand every pregnant and labouring woman is an individual therefore your care provider, who has extensive knowledge of your medical and physical history should always remain your medical advisor.  Participants are always asked to consult with their care provider regarding course information, as there may be variations or exceptions for information contained in the antenatal course that your Doctor or Midwife may recommend based on your individual health circumstances.

The information and educational materials used to facilitate the course are evidenced based, reviewed and updated regularly and are standard for the teaching of prepared childbirth education.  Articles given to participants at the course do not necessarily reflect the opinion of My Midwives but are given to show a range of opinions and views expressed around a number of topics and to promote thought provoking discussion.

In the interest of maintaining good collaborative relationships it is advised by My Midwives that you notify your care providers of your attendance to a childbirth education course.  Your Midwife and Doctor providing your ongoing care should always be consulted for advice regarding any decisions determined for you and your baby.

My Midwives desires that all participants of the course achieve a positive childbirth experience however My Midwives make no claims that participation in the course will guarantee a desired childbirth outcome.

Payment

Payment may be made by EFTPOS or direct deposit.  Please quote your surname when making a direct deposit.  You will be invoiced before the commencement of the course.  Bank details are on the invoice.

** All Course Fees are payable in full before commencement of course.   If the fees are unpaid it is understood participants will be unable to attend the course.

My Antenatal Class

$418 (includes GST)

$250 will be refunded if booking is cancelled with a maximum 2 weeks notice.

New Born Care Workshop

$104.50 per class (includes GST)

$55 will be refunded if bookings are cancelled with a maximum 2 weeks notice.

Declaration

 The content of this course is specifically designed to inform you, your partner or your support person and is not intended to take place of the advice of your care provider, rather it aims to provide you with the necessary information and tools to ask questions, source further information and make informed decisions that are particular for you and your baby.
