406 Consignary Missing Item Form
  • 406 Consignary Missing Item Claim Form

Hey 406 friend! We hate when items go missing and we do everything we possibly can to match sellers and items back up!
  • Format: (000) 000-0000.
  • Current Event Participation Level*
  • And*
  • When did you complete your pick up appointment? With hundreds of sellers and thousands of items- 406 always does their best to rematch unsold items to their owners. The later the pick up appointment, the faster and more accurate your checkout process will be.*
  • When I registered for the 406 Consignary event I signed a form that verified I understand 406 Consignary does not cover theft, loss, or damage sustained to consigned inventory.*
  • Did you check the Missing Tag Station (Lost & Found) The super long tables and racks lined up right next to where you waited for your consignor check...*
  • {{{This is the most common place for items to get left- if your items were here- they're donated Sunday night and you can mark them on your roster as a tax deduction.}}}

  • Did you check ALL the way through your piles WITH a staff member? All of our staff and volunteers do everything we can to make sure items are returned to the correct bin and that everything gets double checked prior to it leaving the building.*
  • This is often the place where a volunteer has good intentions- BUT people accidentially miss 1 item inbetween others. We emphasize the need to double check EVERY SINGLE ITEM... but human error is a real thing. Thankfully if this happens the item DOES get found and winds up on the Lost & Found. (see above) *IF* an item gets all the way home and the consignor lets us know after the fact- we encourage them to bring it back next season and hopefully it'll make its way into your SOLD items next sale- or return home next time.

  • Did you and a staffer double check consignor piles that were neighboring or may have a similar number? ie: 788 vs 778, 1131 vs 1311 can look similar when reading quickly after working crazy busy shifts...*
  • Did you and a staffer double/triple check your consignor number area before leaving the venue? Always do this you never know when something that was misplaced gets returned to your spot (or Lost & Found) after you collected your pile.*
  • Are there any lines in your seller report that say "Non Inventory Item Added At Register" or similar? If a tag doesn't scan - we have to type it by hand, which means the computer auto generates a new tag for an item you already have in inventory... so is there a chance the missing item actually sold?*
  • Are you 100% sure you can verify the item arrived at 406 Consignary, passed inspection, and was placed onto the sales floor? Often consignors forget items that didn't pass inspection aren't in their unsold pile as that item already returned home before the sale days :)*
  • As a reminder, all participating consignors have signed digital or paper copies of our Seller Agreement which states:

    • In consideration for the opportunity to participate in and profit from 406 Consignary Consignment Sales, the undersigned seller agrees to the following: To hold harmless and make no claim against any 406 Consignary Venue, the owners of the space where the sale is held, any volunteer or paid participants in the sale, the Valet Tagger or the insurers of Johanna or 406 Consignary for personal injury or for the damage, theft or loss of any items consigned at 406 Consignary.

    And

    • No claims will be made against the staff, volunteers, owners, or 406 Consignary for any discrepancies involving items placed in the sale.

    And

    • I understand I (the consignor) sigend a contract that stated anything left unclaimed in the building at the end of the night needs to be cleared out and is immediately sent to donation.
  • What would you like for 406 Consignary to do *if* the missing item is found?*
