PARTICIPANT HEALTH HISTORY
At Splash Fit Swim Club, our student's mental and physical wellbeing is the most significant priority to us. All participants with any health issues should see a physician before beginning any kind of physical activity. Please inform us of any current or former health issues that the applicant feels would hinder his/her ability to learn or of any physical complications that can create obstacles during the student's learning experience.

I, ___________________________________________ (the participant) and/or the parent/guardian of the participant(s) herby register for training and certification in the American Red Cross Lifeguard and/or CPR/AED/First Aid Course facilitated by Splash Fit Swim Club Inc. I pledge to obey all the rules and regulations set forth by Splash Fit Swim Club Inc and any of its facilities to promote orderly conduct and to protect myself and others from injury. I recognize that a risk is involved when participating in water activities if participating in the Lifeguard Certification Course, including (but not limited to) injuries (paralyzing and otherwise) and death. I agree that this knowledge requires my adherence of these rules and Splash Fit Swim Club Inc.’s discipline.

I (the participant) and/or the parent/guardian of the participant(s) hereby agrees to participate in the selected Certification Course. And in consideration of accepting entry into Splash Fit Swim Club Inc., I hereby waive and release the business, Splash Fit Swim Club Inc., my Instructor(s), staff/employees, agents, independent contractors, members and authorized guests from all responsibilities and all claims for any injury that I may receive while participating in aquatic activities. I also agree to indemnify Splash Fit Swim Club Inc. and the occupying facility for any damages incurred arising from any claims, demand, action or cause of action by myself or the participant.

I (the participant) and/or the parent/guardian of the participant(s) also agrees to authorize Splash Fit Swim Club Inc. or any representative of Splash Fit Swim Club Inc. to have the participant treated in any medical emergency during their participation in any course or while residing in the facility, and furthermore agrees to pay any cost associated with the medical care and transportation for themselves or the participant(s).

I verify that I have received, read, understood and agree with everything on this registration/release form, which includes the purpose, who the course is designed for, facility information and address, certification details, what I will receive, what materials need to be brought with me, course length, cost, number of students, how to pass the course, prerequisites, and Instructor(s) contact information.

I understand that photos and/or videos may be taken at Splash Fit Swim Club Inc. during a Certification Course and agree to allow Splash Fit Swim Club Inc. to use photos and/or videos of myself or participant(s) for advertising, publicity, their website, and/or training.

I hereby authorize any representative of Splash Fit Swim Club Inc. to arrange to treat myself or the participant(s) for injury in any medical emergency during participation in aquatic activities under their supervision. I hereby waive, release and forever discharge and agree to indemnify and hold harmless Splash Fit Swim Club Inc., any facility in which their services take place, their shareholders, owners, members, directors, officers, agents, independent contractors and employees from any and all actions, suits, demands, claims, damages, attorney fees, and causes of action of any type or kind whatsoever arising out of or caused by myself or the participant(s) in any aquatic activities at Splash Fit Swim Club Inc. I the participant and or parent/guardian am also aware of the non-refundable fee.
