FIT-ABLE Housing Accommodations Renewal Request Form

FIT-ABLE requires students renew their housing accommodations (including Emotional Support Animals) once an academic year at the start of each Fall semester. Please note that if this form is completed after priority deadlines, then your housing accommodation may not be able to be granted; you may be waitlisted.
