  • Please send us a copy of your RRF Post Award Report that was sent to the Small Business Administration (SBA). This is the form that you used to report how much of your RRF was spent on payroll, rent, utilities, etc.

  • This is the last section of the questionnaire. We would like to ask you several questions about the impact of government mandates on your business during COVID.

    The better we understand the business, the more we can MAXIMIZE YOUR CREDIT.

    We understand these questions can seem tedious; you can skip questions and leave fields blank if needed. However, the more information you are able to provide, the more likely our research team can find ways to help you qualify for the credit.
