This judging forms must be submitted at least six weeks prior to the show’s opening performance. Late judging forms may result in judges not being available.

All sections of the judging form must be completed.

Please use the terms "Not Applicable" and "Not to be Judged" where appropriate.

Please use the term "TBA" (To Be Advised) if an area will be judged but the information is still to be decided upon.
