Although the safety of all sport activities is the primary concern, indoor sport activities at Youth Sport Center's facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Hewlett East Rockaway Jewish Center and the persons in charge.

Easy -Payment CenterWelcome... First time attendants can move on to submit this form, as first time visitors attend FREE FIRST SESSION. Returning Athletes are required to submit registration fee prior to March 16. The subscription can be cancelled at any time. If athlete does not continue with the Spring/ Summer/ Fall season The subscription will be cancelled before the next 8-week session begins. In the event a charge is sent before a player has a chance to cancel, the family will receive full refund. After completing one 8-week session, if your Athlete attends any one of the following sessions, it is assumed that you would like to continue with subscription. In that case there is no refund for that session.
