SUMMER CAMP PROGRAM AUTHORIZATION FOR EMERGENCY TREATMENT

If my child should become ill or injured at Mountain Laurel Montessori School or during a field trip sponsored by the school, I understand that the staff of Mountain Laurel Montessori School will:

1) contact me immediately, or

2) contact the person(s) I have designated if I cannot be reached.

Should Mountain Laurel Montessori School be unable to reach me, or the person(s) designated, an MLMS representative is authorized to contact my child's physician and/or arrange for immediate emergency treatment. The physician and/or medical facility are authorized to administer emergency medical treatment necessary to ensure the health and safety of my child.

SUMMER CAMP PROGRAM PERMISSION TO PARTICIPATE IN FIELD TRIPS

My child has permission to participate in all field trips sponsored By Mountain Laurel Montessori School. I understand that the school will notify me in advance of any field trip in which my child will participate.
My child is in Toddler or Primary Summer Camp and does not have permission to attend field trips.

SUMMER PROGRAM PERMISSION TO PARTICIPATE IN WADING

My child has permission to participate in wading pool activities at Mountain Laurel Montessori School. I will supply a dry bathing suit and towel at the beginning of each day, and a bag to put wet clothes in after the activities.

SUMMER PROGRAM SUNSCREEN AND/OR INSECT REPELLENT WAIVER

I understand that it is the family's responsibility to apply sunscreen and/or insect repellent to my child before arriving at camp in the morning.

MLMS will administer a 50 SPF sunscreen spray and a natural and DEET free bug spray from the school's supply, according to the schedule noted below:
- MLMS staff will administer sunscreen at midday before going out to the playground (for Primary students) or before driving to the pool (Elementary students).
- MLMS staff will administer insect repellent before going to the woods or if necessary, during field trips (Elementary students).
If I/we wish MLMS to administer a sunscreen or insect repellent of my/our own choosing, I/we understand that an over-the-counter medication release will be completed by the family, and that those products (provided by the family) will be used in lieu of those provided by the school.

If I/we do not wish for either product to be administered at all, I/we will submit that request in writing.
