Please fill out this form and submit it as this will help me determine how many people are planning on coming to the clinic. Also, there will be a link to the release form on your conformation email. Please download it and either send it back to me or bring it with you to the clinic. I have also included a payment option so you can pay here when you register or you may pay when you arrive at the clinic. Cost of the clinic is $50 per participant. Please check in with one of the volunteers when you arrive, just do not park anywhere. Please, also, have current Coggins for each horse as it will be checked.
