Chair massage request form

REQUEST FORM FOR CHAIR MASSAGE
Yestohealingllc provides the chairs for massages, the therapists and any other supplies needed in regards to sanitizing after each massage.

Chair massages range 8 - 12 minutes long, depending on the flow of the event.

All therapists will be dressed in black with footwear of their choice.

We arrive 30-20 minutes prior to event to set up.
