I hereby give my consent to all photographs and/or audio and video recordings of me and my minor child's voice, likeness, image, or academic work captured by Utopian Academy for the Arts Charter School Network (UAFA) or its designee. I understand that any such photographs, audio and video recordings, and images of academic work will become the property of UAFA and may be used by the UAFA Network or other designee for educational, instructional, and promotional purposesin print, broadcast, and online media formats now existing or created in the future.
I, furthermore, give my consent for news media, and UAFA employees or designees to interview me and my child about school programs. I understand that my child will be under the supervision of a school staff member during such Interviews occurring in my absence. My child reserves the right to refuse to answer any questions or participate in any discussions with an interviewer or terminate a photo session at any time, for any reason. I also understand that neither UAFA, nor the news media, has any obligation to air or publish the footage or voice of my child or myself. I also understand that neither I, nor my child, will receive any monetary compensation for the rights granted herein.
Pursuant to this Media Release, I hereby hold harmless UAFA, its successors, and designees that publish photographs and/or audio and video recordings of me and my minor child's voice, likeness, image, or academic work from any and all claims, damages, liabilities, costs, and expenses which or my child now have or may hereafter have by reason of any use thereof. I understand that the provisions of this Media Release are legally binding, and that UAFA will not be responsible for, and cannot control, photographs, audio recordings, or video recordings captured by guests attending UAFA events who are not employed by, affiliated with, or under contract with UAFA.
Finally, by consenting to this Media Release, I understand that the Family Educational Rights and Privacy Act (FERPA) permits U.S. school districts such as UAFA to disclose information designated as "Directory Information" without further consent of the parent or eligible student. Directory Information is considered as the name, grade level, years of attendance, photo or video of a student-the disclosure of which is generally not considered harmful or an invasion of privacy. Among the purposes that the Directory Information exception facilitates, is the release of such information in official school publications and athletic, extra-curricular, and graduation programs, as well as school yearbooks, and honor rolls/recognitions. Directory Information may also be shared with the PTA and other legitimate, school-approved agencies, entities, and individuals.
